Upper blepharoplasty treats sagging skin on the upper eyelids, while lower blepharoplasty can help with under-eye bags, extra fat, and changes in the lower eyelid skin.

For lower eyelid surgery, Dr. Stong may use fat transposition to preserve and reposition fat instead of simply removing it. In selected patients, a transconjunctival approach places the incision inside the lower eyelid. Medical literature notes that lower eyelid blepharoplasty is technically complex, so surgeon training and patient selection deserve real attention.

Filler can sometimes make under-eye problems worse if the real issue is fat bulging, fluid buildup, or loose skin. Using too much filler may cause heaviness or swelling instead of a smoother look.
